I'd be happy to take a look.  The need fot simplicity, particulaly in
volunteer programs is always an issue.

We did have some success with the following form:

1.  This semester I learned ___

2.  This will help me____

3.  The most intersttin thing we did was____

4.  I am proud of_____

5.  Next semester I hope we_____

6.  I also want to say____

Because this form is filled out by the student, often with assistance
from the instructor or other students, it serves as a lesson as well.
Though I haven't operationalized it much yet, I think processing any form
of  qualitative assessment through instruction is an excellent way to go.
 It may also be a good way to explain EFF to students (that is, making
one or two lessons out of the EFF framework), though I have not yet done
that.
