I am looking for examples of an adult reading and writing screening assessment which is a program intake form. The intake form would be designed to do two things: 1) provide intake information and 2) give a rough assessment of reading and writing skills for placement in the program or class. An applicant would be asked to fill out the intake form in the presence of an intake worker, counselor or teacher, and would be told that s/he could get assistance at any point s/he needed, including right from the beginning. Have you used such an instrument? Do you know of actual examples of this being used somewhere? What do you think of this as a placement tool? Thank you for any assistance you may be able to give me.
