You might think of HeLP (for Health Literacy Program), since it is clearly a program to help people to better access and outcomes in health care. However, HLP does do well.

The Health Literacy Program is currently being called "HLP".  It will serve
economically and academically disadvantaged adults.  The intent is to
expose the participants to low-level health materials and tools that will
assist them in developing skills to becoming comfortable in navigating the
health care system, communicating with health care providers and
understanding medical instructions.  The participant will be taught to seek
health information and resources, interpret and understand this information
and use it to enhance their health.  I hope this additional information
helps.  Would certainly appreciate help in naming the program.
