What a great project! I know there is a need for this kind of resource list. 
I have worked with diabetes issues for several years. There seems to be quite 
a bit of material available at high reading levels. Fortunately, there are 
more and more easy-to-read materials coming out too. 

A wonderful place to start would be the Indian Health Service National 
Diabetes Program headquartered in Albuquerque, New Mexico. They have 
developed a lot of information on diabetes for American Indians throughout 
the US over a period of years. Some of their materials are just 
beautiful--easy-to-read, practical, encouraging, and very culturally 
appropriate. They've also produced many videos, teaching curricula 
appropriate to native peoples, and more. They can tell you which materials 
are available for distribution.
