I think (and certainly hope) that the MIME problem is now fixed. The ordering form is a snafu between the Web developer and my Web hosting company and should be resolved very soon. My apologies. "Partnering with Patients To Improve Health Outcomes" is about a model of care in which patients and providers work together to improve the patient's health.
