Hello,
I am also new to the listserv, and am looking forward to reading it.

Linda, I would be interested in seeing an example of the patient discharge
form, before and after, as a practical example to show my coworkers
(obviously with any confidential information either omitted or new
information fabricated, if it was for an actual patient). As this may be
"old news" to other listserv members, feel free to respond directly to me.

Debbie,
I appreciate your question on the quietness of the listserv.  I read it like
crazy, but I don't often post responses or questions.  I miss the activity,
but trust that it is just a temporary lull.

 I would like to share that just yesterday my co-worker and I stirred some
excitement on a patient discharge form that we were given permission to
re-do with health literacy in mind.  It was very well received.  We are very
encouraged.
