This project will investigate the barriers to, and strategies for, successfully implementing the NQF-endorsed informed consent practice: ask each patient or legal surrogate to recount what he or she has been told during the informed consent discussion. The project will focus on patients undergoing non-investigational, invasive procedures in either inpatient or outpatient settings and have a particular emphasis on providers serving large low literacy and LEP populations. The project will include: 1) detailed self-assessments by a few collaborating "early adopter" providers who have implemented the practice, based on their experiences in general and with low literacy/LEP patients; 2) interviews with "non-adopting" providers that have not implemented this practice but would like to do so; 3) an analytic case study describing and comparing the collaborators' experiences; and 4) an interactive workshop held in the fall of 2004 to discuss and expand on the experiences that includes the collaborators, other providers, and consumer and purchaser representatives. The project will be overseen by a Technical Advisory Panel and culminate in workshop proceedings, including the analytic case study, and a user's guide designed for providers who wish to implement the practice. We are seeking providers for the first two tasks of the project, particularly for item 1. Despite the importance of appropriate communication practices to ensure fully informed consent in medical decisionmaking, there do not appear to be many providers (facilities or individuals) who have formally implemented the "repeat back" informed consent practice as defined by NQF, and any information listserv participants have on examples of this practice being conducted in healthcare facilities would be welcome.
