And if I can extend this a little more --- the need is not just for a form
on each individual student, but on the issues of systemic change needed so
the student can get what they need.  

So -- as far as a form goes - the best think I have seen has been developed
by the VR system in Alabama, through their state wide effort transition
efforts.  Linda Hames is the lead person in the state on that -  (they
operationalized the VR definition of LD and developed a six page form for
assessment needs.) Excellent work - but it may be too extensive for many
programs.  And since the form is filled out based on extensive testing ...
it may be too expensive of a model  for some - But when you asked the
question - you did not ask about costs.

Now for systemic change - the Briges HUBS and myself have developed a seven
step model that consists of "screening, diagnostic testing, empowered
casemangers (or teachers),  disability partnerships, trained employers,
trained consumers (on lots of things but especailly on defintion of LD and
accommodation options) and on-going supports.)  Look at www.seakingwdc.org
under the LD section on systemic change - there should be the "report card"
we developed for program evaluation there too.

Thanks Glen, I certainly understand the points that you've made. Let me see
if I can be a little more specific in my request. I'm looking for a "form"
which programs can use to write individualized programs for adults in adult
education programs. This "form" would indeed be the statement of necessary
accommodations that adults require in order to be successful in his/her
program. 

Is anyone out there using such a "form"? Might you share this form with me?
