It should be possible for you to get a copy of that instrument as well as the report summarizing the results. It was part of what was then known as the "Capacity Building" initiative. I believe it was conducted by the DC firm TATC and that the ETA project officer was Dolores Beran-Hall (202-219-6236 ext. 149). If you have trouble getting copies, give me a call.
