Thank you for your interest in partnering with El Paso Community College/Community Education Program to disseminate a health literacy model with funding from Pfizer. Below are the details of how I am planning to proceed. Please read careful. Partnering programs will not be receiving any money to implement effort. However, they will receive free training on how to implement a health literacy model in their community. Specifically, in the application I will write that representatives from our program will travel to your community and conduct training so that a health literacy program can be developed and implemented in your community. Your program will have to agree to identify one or more teachers/tutors/educators, a group of students and one or more health organizations (i.e.: County Hospital, Health Clinic, Planned Parenthood, etc.) for this project. We will develop a training model to work with the teachers and the partnering health entities. We will spend a couple of days training the teachers and representatives of health entities. The teachers, with support of the health agencies, will conduct 12 to 24 hours of classroom instruction incorporating relevant health topics into the ongoing classroom instruction (GED, ESL, literacy) They teachers/educators/tutors will have to conduct some kind of an assessment of the health services delivered and provide a summary of some sort. Students receiving health instruction mus t fall in one or more of these categories: Low literacy Low income Elderly Culturally diverse This is the general premise of the proposal that I am planning to write. This model may not fit everybody’s plans. If you have some other ideas that could fit into this general scheme of receiving some kind of training in your community, please let me know. I will not begin writing the proposal until January, but I want to get confirmation of your intentions no later than December 12. If you want to discuss the proposal further, please let me know. Please remember that there are several entities interested in the partnership. Therefore, I want to submit a proposal that meets the specific needs of each community, yet keep it as uniform as possible between communities. This will make the effort easier to implement and assess. If you decide that you want to participate in this effort, please confirm as soon as possible. We will be closing for Christmas on December 14, and we will not open until January 2 Your agency will need to write a letter of agreement. I am including a sample letter. Please modify it accordingly, but be specific as to the things that you will agree to do. Sample: Andres Muro, Coordinator El Paso Community College/Community Education Program, 1115 N. Oregon, El Paso, TX 79902 Dear Mr. Muro: (Name of you agency) supports El Paso community College/Community Education Program’s (EPCC/CEP) application for a Pfizer Foundation Health Literacy Community Grant to disseminate a health literacy program model in our community (name of community). There is tremendous need in (name of community) to establish a health literacy program (Please describe your need). We will partner with EPCC/CEP to make this application a complete success. Specifically, our program will do the following: 1. Our program will identify xxx teachers/tutors/educators to participate in training conducted by EPCC/CEP to develop and implement a health literacy program in our community. 2. Our program will identify representatives of (name of one or more health agencies, such as county hospital, health clinic, state department of health, city/county health department, red cross, other) to partner with us and attend training conducted by EPCC/CEP. 3. Our program will identify a minimum of xxx many students to attend 12 to 24 hours of health literacy instruction 4. Our teachers/Educators/Tutors will work with representatives of local health entities to incorporate between 12 and 24 hours of relevant health information into our ongoing instruction. 5. Our teachers will assess the impact of the instruction to their students. The teachers will assess both gains in health related knowledge and language skills, and will provide a summary of assessment results. 6. Our program will provide a summary of the number of students served and their demographic characteristics. Please fund this very necessary effort. Our community will benefit greatly form this.
