Don't know the publisher right off hand, but can find out if you wish. In a different area, we use an instrument called Working to assess nine aspects of work ethic/soft skills: responsibility, teamwork, quality, systems thinking, desire to learn, problem-solving, skill at learning, adapability, and persistence.
