Dear subscribers,

The NIFL electronic discussion groups have been in operation since 
May 1995. NIFL-workplace now has over 700 subscribers! I conducted a 
subscriber survey on the one-year anniversary of this group but 
haven't done one since then. So, on our five-year anniversary (close 
enough!), we would like to find out if this discussion group is 
meeting your needs and interests. We will post a summary of the 
survey to the list.

The survey should take only about 10 minutes (maybe less!) to fill 
out. Individual responses will be kept confidential -- as long as you 
follow the return directions below. If you return your survey to the 
subscriber list, your responses will be broadcast to all 
NIFL-workplace subscribers.

After you have completed the survey below, please e-mail it to 
BLV1@PSE.EDU, fax it to 814-863-6108, or snail mail it to Barbara Van 
Horn at 102 Rackley Building, University Park, PA 16802-3202.
