Daniel Some things I'd check: 1) the readability. I've seen a number of forms that weren't at the parents' level. 2) The function-will this form give you the information you need? 3) How inclusive is the form for all the families? This is the start of a relationship and being as inclusive as possible from the get-go can increase your retention and enrollment.
