commitment to health literacy" might want to check out:

I am going to be using a version of their survey at an upcoming conference to
measure healthcare professionals' knowledge, attitudes, etc. regarding health
literacy (with Sue Graham's permission--she has been easy to work with and very
generous with the instrument).
