Matt, Congratulations on making a real effort to reach out to low literate patients! A quick look at the patient information brought two things to mind. One is that the print font is san serif which is not as easy to read for a low literate person than a serif font. Secondly, the readability level is that of a seventh grade plus. I would rewrite this to make it on a fourth or fifth grade readability at most. You might want to have an adult education class take a look at the information to see if it is clear to them.
