Some of may recall that I was working on creating patient education forms. The concept is to treat patient education as a continous process that should be documented, evaluated, followed up and build upon. Please see the website www.healthliteracynow.org
for the forms. Please feel free to use it for your patients as long as you reference the website and provide me the name of your institution (if you have not done it before).
