My favorite "from the ground up" writing program is "When They Can't Write" by CHarlotte MOrgan (http://www.proedinc.com/store/index.php?mode=product_detail&id=12057 ) It starts with subject/predicate sentences... no, it starts with what's a noun and what's a verb, tho' it's very example oriented as opposed to "do you remember what this is called?" oriented. Students learn different sentence structures and get lots of practice that's reasonably creative.
