This may be more than you're looking for and is only adult, but the
Illinois Adult Education Service Center Network developed an adult
education teacher self-assessment of skills and knowledge which can then
be used to determine professional development needs; the self assessment
is part of a Professional Development Planner. There is one Planner for
ESL and one for ABE/GED. They can be found and downloaded from
http://www.thecenterweb.org/alrc/admin.html.
