I have only conducted one-time interactive audio-video teleconference
workshops, not classes taught on a regular basis. I've done research on
distance learning and the following principles apply:
--Classes should total no more than 30 including all sites; the best
learning usually occurs with no more than 3 sites.
--A best practice is to have a "site coordinator"--someone who calls
the class to order, gathers any questions before and after the session
in order to share them with the instructor, and may also pass out
materials, like handouts, quizzes, etc.
--The class should be structured so that there is interaction between
the instructor and each site. Maybe have different sites answer
different questions. A best practice is to rotate sites, i.e., the
instructor broadcasts live from a different site each time.
--Distance learning theory talks about three types of interaction
referened--interaction with the instructor, interaction with the
material and interaction among students--learning takes place through
all of these.

I hope it helps.

My question was more on how to make the technology work better for me,
the teacher, and my students. I enjoy having pair and group activity,
but it is difficult when half the class is in a room in another city.
And I worry they are not having fun with the activities like my students
right there with me, which I can observe they like the activities and
are learning.
Also, I was hoping there was an instructor that had had a class in this
manner to give some helpful pointers.

What about the possibility of having the students "call" you on Skype
to practice their speaking skills?
